Mental Health/Behavioral Health Insurance Benefits Verification Form

Mental Health/Behavioral Health Insurance Terms and Definitions

It is extremely important that you find out exactly what behavioral health services your insurance policy covers. You can do this by reading the section in your employer’s insurance coverage booklet that describes behavioral health services, logging in to your online insurance portal, or by calling the number on the back of your insurance card and speaking with a representative about your benefits. Please refer to the Mental Health/Behavioral Health Insurance Benefits Verification Form by clicking the link below in order to determine the cost of your sessions and the details about your benefits.

Mental Health/Behavioral Health Insurance can be difficult to understand and frustrating to navigate. What is the difference between co-insurance and co-pays? How does a deductible work? Why is the amount charged by my counselor and the amount allowed by my insurance company so different and what does that mean for me? For answers to these questions, and more, please refer to the Insurance Terms document by clicking on the link below. If you have further questions, please contact your insurance company or call us at 740-990-8270. We will do our best to answer your questions!
